Templates is an area of the platform where templates for different types of reports can be created. Access to view, add, and edit the templates within this tab is a permission that can be set within Roles.

Within the Client Report Templates tab, templates can be made for:

  • Treatment Reports

  • Billable Session Notes

  • Non-Billable Session Notes

  • Behavior Intervention Plans

Each template TYPE is then used within different areas of the platform:

  • Treatment Report templates are used for writing treatment reports for Clients. These templates are pulled up and used in the Funder Report tab of a Client’s profile, thus creating a treatment report for said client (E.g. progress reports, re-authorization reports, etc.)

  • Billable Session Note templates are used for completing session notes for a client following a billable session.

  • Non-Billable Session Note templates are used for completing session notes for a client/task following a non-billable session.

  • Behavior Plan templates are used for writing custom Behavior Support plans in the Behavior Support tab of a Client’s profile.

To begin creating templates, select Templates:

Select Add New Client Report Template:

Select the Form Type, Funder and Template Name of your choice:

  • Form type is choosing from the four options (Treatment Report, Billable Session Note, Non-Billable Session Note, and Behavior Plan)

  • You can add one or multiple funders to the report form type. Pairing funders to the template allows the template to be used with a client who has that funder.

    • Example: Client A has Blue Cross Blue Shield. If a treatment report template is not paired with BCBS, that Treatment Template can’t be used to run a report for Client A.

  • Name the template something easily identifiable for all staff (Do not use client specific information as templates are used for the entire company)

Select Create Funder Template:

This will bring you to a blank page to build the template. Use the options on the right side to drag and place onto the template.

  • Expand each section to the right to see further options to use within the template.

  • Most options will stay the same, however, some options are specific to the type of template being created.

  • Copy of Billable and Non-Billable Session Note Templates: These templates are made so that clinical staff can complete a session note at the end of a scheduled Billable or Non-billable appointment. These reports are then completed at the appointment level.

  • Creating a Treatment Report Template - These are made so that clinical staff can write a Funder Report (i.e. authorization report, progress report, etc.). These reports are then completed in the Funder Report tab of a client’s profile.

  • Creating a Behavior Plan Template - These are made so that clinical staff have a customized template for information in a written behavior plan. Rethink also has a prepopulated template for behavior plans, so this type of template often is not needed. These reports are then completed in the Behavior Support tab of the client’s profile.

If you’d like to use the sample templates that have been provided in the templates area for you, you can assign your funders to those existing templates once your funders are in the system.

  • You can also edit, duplicate and rename those sample templates if appropriate (all done via the lightning bolt to the right of the template)

To assign Templates to funders:

  1. Select the lightning bolt next to the template you’d like to assign

2. Select Manage Funders

3. You can select one funder or multiple funders from the list that populates by using “ctrl”

NOTE: If no funders populate besides “sample funder” that is because your company’s funders are not in the Company Account tab just yet. This is a prerequisite step to assigning templates.
