Note: Step 1 (Define Behavior) and Step 5 (Track Effectiveness) are the only REQUIRED steps to collect intervention data on a specific behavior.

Step 6 (Evaluate Plan) is an OPTIONAL field and is not required for staff to collect baseline or intervention data on the behavior. By default, all plans are flagged for review 30 days after their START DATE (which was set in STEP 1)

Once Steps 1 has been completed, click EDIT next the Step 6.

In Step 6, Evaluate Plan, you can note which staff are responsible for implementing the plan and recording data on the plan.

Staff do NOT need to be listed here in order to VIEW the behavior support plan and collect data on the behavior.

The staff that appear in the dropdown menu are the staff that have been assigned to the client.

If you wish to add staff that do not appear in the dropdown, type into the SEARCH box, then click ADD.

When editing Step 6, notes can be left regarding the behavior and current plan.

Behavior notes can also be left via the VIEW PLAN tab:

Any Behavior Notes can be viewed at the bottom of the behavior support plan via the VIEW PLAN tab,

A specific review date can also be selected.

A red flag will appear next to the behavior upon the review date.
