Elementary Classroom Rating Scale

We need to add a SEL Classroom Rating Scale for students in grades 3-5. This will function in a similar manner to the existing rating scale, but with different questions, scoring, and business rules.

User Stories:

• As a teacher, I want to assess my elementary students on a series of items, so I can better understand where their strengths and weaknesses lie.

• As an administrator, I want to view a report of my elementary students, so I can understand how our students are performing.
