Client Demographics is the area that contains required and optional fields when adding a new client into Rethink. This resource explains all of the fields in the Demographics tab of a client’s profile.

Rethink Client ID	This field is a unique identifier for the client created by Rethink The number will auto populate after the client demographic information is saved Pulls into the Billing Export Report
Client ID #	Optional A placeholder in the system allowing companies to add their own internal client ID
UCI #	Optional The Unique Client Identifier number is used mainly by California providers in order to pull on the Parent Verification Form needed for California Regional Centers
Client Order	Optional Allows companies to assign an order number to a client and search/filter client list by this number
Reason for Referral	Optional Free text area for internal documentation regarding reason client was referred for services
Notice of Privacy Practices Provided to Client	Optional Date that privacy practices, HIPAA compliance procedures were shared
Location/Facility	Required Client can only be assigned to one location Locations created in the Company Settings
Status	Required Client must be set to an ACTIVE status to have full access to programming, data collection, and scheduling
Service Intensity	Required INTENSIVE—unlimited number of skill acquisition and behavior reduction programming can be added to client NON-INTENSIVE—limited number of skill acquisition and behavior reduction programming can be added to client (5 total…2 of which can be behavior reduction)
Kareo Patient ID	BILLING CUSTOMERS: The ID number assigned by Kareo
