Creating Funder Reports.

Funder Reports is a location within a Client’s profile where treatment report writing is completed for the client. (E.g. Progress Reports, Initial Authorization reports, Re-Authorization Report, etc.)

Funder Reports are created from treatment plan templates made within the Client Report Templates area. The way in which Funder Reports are written is specific to the way in which the template was built and formatted. This resource will review those fields in more detail as they relate to completing a report

Generating a Funder Report

 

When ready to create a Funder Report for the client, you’ll navigate to the client’s profile and select Funder Reports

Select ADD FUNDER REPORT

 

Select from the TEMPLATES provided within the drop down menu.

Select the SERVICE LINE

Select CREATE FUNDER REPORT

 

Complete the Funder Report by filling out the sections within the template

NOTE: The sections displayed within the Funder Report were based on how your administrative team created the template. There will be several fields that pull in data regarding the staff, client and organization automatically, that is per the template, but there will be other fields that require further completion/anecdotal information.

Pulling in Programs and Clinical Data into a Funder Report

There are four fields that can exist within a template that allow programs to be pulled in. Depending on the field that is placed in the template, it will pull different types of programs or from different areas of the client’s profile. See the options below to identify the functionality of each option.

Skill Acquisition Progress

 

 

 

 

  • If this option is used in a template it will pull from all skill acquisitions programs located within the Current Tab of the Client Program section

  • After typing in the dates and completing the filter options, select Preview and all programs will pull in

Once preview is selected, program information and graphs will pull in to display data based on the filters and dates chosen.

  •  

  • Graphing Filters available for display:

  • Individual filter to each graph display can be made even after selecting preview

  • A graph title (name of the program) will default to the top of the graph and can be edited as needed. There is no character limit for chart titles in funder reports.

    • For target-specific graphs, one title can be used for all individual graphs that are target specific

  • This field is only placed in a template once as it pulls in all programs within the current tab of a client’s program section. By deselecting “Include in report” an individual program can be removed from the report

Behavior Reduction Progress

 

  • If this option is used in a report, all behavior plans with data collected will pull in

  • All details that were placed within the behavior plan within Behavior Support will also display

  • Graphing Filters available for display:

  • A graph title (name of the program) will default to the top of the graph and can be edited as needed. There is no character limit for chart titles in funder reports.

    • For target-specific graphs, one title can be used for all individual graphs that are target specific

  • This field is only placed in a template once, any behavior plan that pulls in that is not preferred to have on the completed report can have “don't include on report” selected

Recommended Skills

 

  • If completed, this field will pull from the Future Tab of the Client Program section. These goals typically do not have data collected as they are recommended for further intervention

Recommended Behaviors

  • This field pulls from behavior plans in the Behavior Support section that have Step 1 completed, but do not have any data collected

 

Using Custom Graph and Custom Table Fields

If Custom Graph option is added to a report template, outside files can be uploaded to include into the report

Saving, Downloading and Editing a Funder Report

Once all fields of the Report are completed, select SAVE.

  • You can always edit the Report further if needed (add to it and save a new version)

  • If fields with asterisks are not filled out, draft versions will not save

Note: the report automatically saves every few minutes, so the Save As option only populates if a version has been automatically saved or previously manually saved

Once SAVED, a report can be:

  • Further Edited (EDIT FUNDER REPORT)

  • DOWNLOADED (into a word document or a PDF)

  • SAVED TO FILING CABINET

NOTE: If a report is saved to a word document, FURTHER EDITS can be completed outside of the Rethink platform

  • Examples: Adding outside assessment information to the report, editing fonts, adding in additional visuals or graphs, etc.

 

Additional actions may be completed from the Funder Report landing page (depending on permissions). This is done via the lightning bolt next to a listed report.
