Client Demographics Fields Explained
Client Demographics is the area that contains required, and optional, fields when adding a new client . This resource thoroughly explains each option available when adding a new client.
IMPORTANT: Some options explained below are ONLY available based upon the products and services your organization uses. Those will be noted accordingly.

Client ID # | Optional A placeholder in the system allowing companies to add their own internal client ID. |
UCI # | Optional The Unique Client Identifier number is used mainly by California providers in order to pull on the Parent Verification Form needed for California Regional Centers |
Client Order | Optional Allows companies to assign an order number to a client and search/filter client list by this number

Reason for Referral | Optional Free text area for internal documentation regarding reason client was referred for services |
Notice of Privacy Practices Provided to Client | Optional Date that privacy practices, HIPAA compliance procedures were shared |
Location/Facility | Required Client can only be assigned to 1 location

Status | Required Client must be set to an ACTIVE status to have full access to programming, data collection, and scheduling |
